Registration Information
Inpatient and outpatient registration hours of operation are from 7:00AM- 6:00PM. Emergency registration is available 24 hours a day, seven days a week. To pre-register for scheduled services call 334.222.6937. Andalusia Regional Hospital has contracts with most insurance carriers. Please check with your insurance carrier for more information.
Inpatient Registration Information
You may be required to sign the following:
* Consent for Treatment
* Authorization to bill insurance
If you are a Medicare Recipient, you will be required to sign:
* Medicare Secondary Questionnaire
* Medicare Rights Statement
Remember to bring with you at the time of registration the following items:
* All active insurance cards
* Picture ID
* Complete list of current Medications
* Living Will, if you have one
Patients are encouraged to leave valuables at home to ensure safe-keeping.
Outpatient Registration Information
Upon arrival, go the Admitting Office located in the front Lobby of the hospital to register. It is important that you arrive on time, because your physician may have scheduled special tests for you. We are required by law to ask if you have an Advanced Directive.
